ALEXANDER PERALTA, Jr. RPh, MD, DABHPM

Dr. Alexander Peralta was a registered pharmacist from 1968 to 1975 and has been a physician since 1980. In 1982 he moved to Duncanville, Texas and practiced acute care and general internal medicine for several years. He has served two hospital systems in several capacities including Department Chair, Chief of Staff and as a trustee. In 1992, he accepted a full time position as Chief Medical officer for Family Hospice, Inc. Other positions include Associate National Medical Director for VistaCare, Vice President of Medical Services of Community Hospice of Texas, President of Community Center for Palliative Care, Director of Palliative Care Services and Chief Medical Officer for American Hospice, Inc., Director of Palliative Care and Education for Universal Health Services and Deputy Director of the University of North Texas Health Science Center at Fort Worth Palliative Medicine Fellowship Program at Universal Health. He is a Diplomate of the American Board of Hospice and Palliative Medicine, certified in 1997 and recertified in 2006.

He has served on international, national and state boards focused on hospice and palliative care and most recently served on the board of the Texas Partnership for End-of-Life Care (TxPEC.org) and currently serves on NHPCO�s Access and Diversity Advisory Council, Diversity Steering Committee and NHPCO�s National VA Advisory Council.

Dr. Peralta has presented at international, national, state and local symposia on topics related to medical ethics, hospice and palliative care services, regulatory and compliance issues, institutional diversity, cultural competency, pain and symptom management with expertise in signs and symptoms at the end of life. He is a member of the National Hospice and Palliative Home Care Speaker�s Bureau (NHPHC.org).

He has served as clinical faculty for Family Practice residents and geriatric fellows at UNTHSCFW and St. Paul Hospital/UTHSCD Internal Medicine residents. In 2004, he was selected to serve on the Texas Department of Health and Human Services� Aged and Disability Committee by the Deputy Commissioner of TDHS. Dr. Peralta was named �A Champion in End-of-Life Care� by the Texas Partnership for End-of-Life Care for 2005.

Memberships include: AMA, TMA, DCMS, American Academy of Hospice and Palliative Medicine, National Hospice and Palliative Care Organization, International Association for Hospice and Palliative Care, Texas Academy of Palliative Medicine, Texas and New Mexico Hospice Organization. He is a Fourth Degree Knight in the Knights of Columbus.

Dr. Peralta is the Chief Operations Officer for Peralta & Peralta Enterprises and President of Palliative Medicine Consulting Services, a subsidiary of Peralta & Peralta Enterprises.
